Step 2 - Membership Registration Form
Directions: To process your membership, complete this form and submit the appropriate payment. You can pay through the PayPal or send a check. (Details above in step 1) A copy of the completed form will be sent to the email address in the form. Be sure to keep a copy for your records. (Please Note: NYALTCA membership is an INDIVIDUAL membership, NOT A FACILITY membership)
